ASEBP's Vision Care plan provides coverage for a variety of services designed to help you maintain healthy eyes. Covered services include:

• Eye exams
• Eye glasses
• Contact lenses
• Corrective eye surgery

Vision Care coverage is available in both single coverage (only you, the ASEBP covered member) and family coverage (for you and your spouse or dependents).

Plan Options Available:

Vision Care Plan 1

• $150 on a rolling two-year basis per covered person

Vision Care Plan 2

• $250 on a rolling two-year basis per covered person

Vision Care Plan 3

• $350 on a rolling two-year basis per covered person
